Tooth extraction is needed when your teeth is grossly decayed or fractured beyond repair or is extremely loose and mobile. Your dentist will only remove your teeth if it is not possible to be restored by a filling or crown. Extraction procedures can be either simple or require some amount of moderate bone cutting (Surgical Extraction). Simple extractions are done in a gentle manner, by loosening the teeth from the socket and removing it with an extraction forceps. Surgical extractions are usually done for wisdom teeth removal. Surgical extraction of wisdom teeth is usually done if there is no space for your erupting wisdom tooth which causes frequent pain and gum inflammation. These teeth usually erupt between the age of 18-23.
An impacted wisdom tooth may:
- Grow at an angle toward the next tooth (second molar)
- Grow at an angle toward the back of the mouth
- Grow at a right angle to the other teeth, as if the wisdom tooth is “lying down” within the jawbone
- Grow straight up or down like other teeth but stay trapped within the jawbone
What are the potential problems de to impacted Wisdom teeth?
Impacted wisdom teeth might result in problems such as:
- Pain around the site of the erupting tooth.
- Trapping food and debris behind the wisdom tooth leading to dental decay of the adjacent good second molar.
- Infection or gum disease (periodontal disease) which can lead to “pericoronitis” or gum inflammation.
- Tooth decay in a partially erupted wisdom tooth
- Damage to a nearby tooth or surrounding bone
- Development of a fluid-filled sac (cyst) around the wisdom tooth
- Complications with orthodontic treatments to straighten other teeth. The pressure can cause some teeth to crowd out.
How is Wisdom teeth removed in a Dental Clinic?
During wisdom tooth extraction, your dentist or oral surgeon:
- Your dentist administers local anesthesia to numb the area around the surgical site.
- Makes an incision in the gum tissue to expose the tooth and bone
- Removes bone that blocks access to the tooth root
- Divides the tooth into sections if it’s easier to remove in pieces
- Removes the tooth
- Cleans the site of the removed tooth of any debris from the tooth or bone
- Stitches the wound closed to promote healing, though this isn’t always necessary
- Places gauze over the extraction site to control bleeding and to help a blood clot form
Teeth extraction are usually needed in the following situations.
- Extra teeth which are obstructing other teeth from erupting.
- Baby tooth which does not fall out even after it is due to exfoliate.
- Wisdom teeth which cause pain and are infected.
After your teeth is removed, your dentist will place some cotton gauze and ask you to bite. You must keep the cotton for 30 minutes to stop the bleeding from the extraction site. Avoid rinsing your mouth frequently as it will disturb the blood clot and will delay the healing. Avoid smoking for at least 3 days after your wisdom tooth extraction is done, as it can lead to a painful condition known as “dry socket”, which can persist for a few days and is totally avoidable.
You must remember to take your medications, which has been prescribed by your dentist or oral surgeon. You can place ice packs on the outer cheek surface intermittently, immediately after your extraction procedure to reduce the post operative swelling. After 24 hours, you can rinse your mouth with luke-warm saline water to rinse away the food debris accumulating in the extraction site. If sutures have been placed after your wisdom tooth removal, you must schedule a follow up appointment after 7-10 days for the suture removal procedure. Your dentist will evaluate the healing of the extraction site and will advise you if further follow up is needed. If you strictly follow the instructions of your dentist, the socket or extraction site would have completely healed by the second week.
